S bolavým břichem se za život setká úplně každý člověk, a to jak v dětství, tak i v dospělosti. Ve valné většině případů se jedná o dočasný stav způsobený nějakou dietní chybou. Někdy však mohou být na vině mnohem závažnější příčiny. Pokud vás bolí břicho na levé straně, může se jednat o projev nějaké nemoci, která postihla orgány, jež se v této části těla nacházejí.

Pokud se u vás projevily nečekané intenzivní bolesti břicha na levé straně, pak se může jednat o závažný problém, který vyžaduje odbornou pomoc, ale stejně tak může jít pouze o nepohodlí způsobené nadýmáním. Pro stanovení přesné diagnózy je potřeba především vědět, jaké orgány se v této části těla nacházejí. Levou stranu břicha je podle uložení životně důležitých orgánů možné rozdělit následovně:

  • Levá horní část břicha: tato oblast zahrnuje žaludek, slinivku břišní, slezinu, část tenkého střeva, levý lalok jater a levou stranu tlustého střeva.
  • Levá dolní část břicha: v levé dolní části břicha se nachází močovod, tenké a tlusté střevo a u žen levý vaječník a děloha.

Vzhledem k tomu, že se na levé straně břicha nachází více orgánů, potenciální příčiny bolesti mohou být různé. Laik tak obvykle není schopen rozeznat, jestli se jedná o zánět slinivky břišní, nebo jde třeba o žaludeční vřed. Pro komplexní a přesnou diagnostiku je proto nezbytná konzultace s lékařem, který vám doporučí vhodná vyšetření. [1, 2, 3]

Co způsobuje bolest levé strany břicha?

Bolest v dolní části břicha je často vyvolána v důsledku plynatosti, ale může být také příznakem vážnějšího stavu, jako je například onemocnění střev, zánět slinivky břišní, kýla a podobně. S těmito potížemi se navíc obvykle potýkají pacienti, kteří trpí Crohnovou chorobou nebo ulcerózní kolitidou. Kromě toho může bolesti vyvolat také alergie na lepek či laktózu.

Velice běžná je pak i bolest břicha po jídle. Zejména po konzumaci mastného, tučného nebo celkově těžšího jídla se mohou objevit nepříjemné bolesti a křeče v břiše. Ty ovšem mohou mimo jiné souviset také s výskytem žlučníkových kamenů (bolesti v souvislosti se žlučníkovými kameny se zpravidla objevují pod pravým žeberním obloukem).

V takovém případě je nutné upravit jídelníček pacienta a zaměřit se na dodržování správných stravovacích návyků. Jídelníček by měl být pestrý, vyvážený a obsahovat zdravé potraviny, včetně potravin s vysokým obsahem vlákniny. Doporučuje se do stravy ve větší míře začlenit ovoce a zeleninu nebo nízkotučné potraviny, vhodné jsou ale i škrobové sacharidy, jako je chléb, rýže, těstoviny nebo brambory.  

Potíže se však mohou někdy vyskytovat také bez jakékoliv zjevné příčiny. Jejich původcem může být stres, deprese, nervozita, úzkost či strach. Takové bolesti se projevují sevřením v oblasti žaludku nebo celého břicha a bývají vázané na konkrétní stresové podněty nebo období. Zároveň souvisí také s nepravidelnou stravou a celkově nevhodným životním stylem. Výsledky vyšetření jsou v tomto případě často nejednoznačné a lékařům může trvat delší dobu, než objeví pravou příčinu obtíží.

Bolest břicha vlevo pod žebry

Bolest břicha na levé straně pod žebry (bolest pod levým žebrem) může způsobit chronický zánět slinivky břišní, vřed žaludku nebo dvanácterníku (zpravidla spíše na pravé straně břicha) či porucha sleziny. Velmi silná a náhlá bolest v levé části břicha ohlašuje akutní problém, který ohrožuje život pacienta a ve většině případů se řeší hospitalizací.

Bolest břicha na levé straně dole

Bolesti ve střední či levé části podbřišku pak mohou signalizovat onemocnění tlustého střeva a konečníku nebo také akutní gastroenteritidu (střevní chřipka). Kromě toho jsou často spojené také se zánětem močového měchýře a močových cest či pohlavního ústrojí, přičemž u mužů bývá postižena prostata a ženy naopak trápí zánět vaječníků, dělohy či děložního čípku. Akutní gynekologické bolesti mívají charakter náhlé příhody břišní. [4, 5, 6, 7, 8]

Nadýmání

Nejčastějším a nejméně závažným důvodem, proč dospělé i malé pacienty trápí bolest na levé straně břicha, je plynatost. Příčinou vzniku tohoto stavu je hromadění plynů ve střevech a žaludku. Obvykle přitom tento problém souvisí s konzumací některých potravin a nápojů, jako jsou například mléčné produkty, sycené nápoje, fazole, květák, brokolice a zelí.

Za hromadění plynů může být však zodpovědné třeba také hltání při jídle nebo žvýkání žvýkaček, kdy dochází k polykání vzduchu. Někdy jej však způsobují závažnější problémy, mezi které patří žaludeční infekce nebo různá chronická onemocnění (například syndrom dráždivého tračníku nebo celiakie). Ve většině případů však nahromaděný plyn během několika hodin sám z těla odejde.

Dopomoci tomu můžete tak, že omezíte spotřebu potravin vedoucích k nadýmání. Nicméně by nebylo vhodné je z jídelníčku zcela odstranit, jelikož se v nich vyskytuje řada esenciálních živin. Jak už bylo uvedeno, jedná se například o sušené ovoce (rozinky a švestky), mléčné výrobky, semínka, luštěniny či jisté druhy zeleniny (zelí, růžičková kapusta či lilek). [9, 10, 11]

Zánět slinivky břišní

Jedním z nejzávažnějších problémů, který vyvolává bolest v levé části břicha (nejen), je zánět slinivky neboli pankreatitida. Slinivka břišní je velká žláza nacházející se za žaludkem a vedle začátku tenkého střeva (dvanáctníku). Její hlavní funkcí je přitom uvolňování trávicích enzymů do tenkého střeva a inzulínu a glukagonu do krevního oběhu.

Pankreatitida může být buď akutní, nebo chronická. V případě akutní varianty se jedná o náhlý krátkodobý zánět, který může pacienta ohrozit na životě. Chronická pankreatitida je oproti tomu zánět dlouhotrvající a obvykle nastává v důsledku předchozí akutní epizody nebo následkem dlouhodobé konzumace velkého množství alkoholu. [12, 13]

Peptické vředy

Další vážnou příčinou bolesti levé strany břicha může být výskyt peptických vředů. Ty se vytvářejí buď v žaludku, nebo v dvanáctníku, což je horní část tenkého střeva. Vznikají v důsledku rozleptání sliznice žaludečními kyselinami. Jejich přítomnost se nemusí nijak projevovat, nicméně většinou způsobuje právě bolesti břicha.

V případě žaludečních vředů se bolesti většinou dostavují bezprostředně po jídle. U vředů nacházejících se ve dvanáctníku pak pocítíte nepohodlí naopak ve chvíli, kdy nejste najedení. Peptické vředy mohou prasknout a vést k vnitřnímu krvácení, které může být pro pacienta fatální. Proto byste v případě opakujících se bolestí břicha měli vyhledat lékařskou pomoc. [14, 15]
