Mravenčení v prstech po ránu či brnění přeleželé nohy vypadá jako banalita. Tyto potíže byste ovšem neměli podceňovat, pokud přetrvávají delší dobu a přidávají se k nim i další příznaky. Na vině mohou být i závažnější onemocnění, jako jsou syndrom karpálního tunelu, snížená funkce štítné žlázy, roztroušená skleróza, infekce či nádorová onemocnění.

Pocit brnění či mravenčení končetin (parestézie) je dost nepříjemný stav, se kterým se ale čas od času setká asi každý z nás. Typicky se objevuje například při přeležení či přesezení dané končetiny. Nejčastěji popisovaným jevem bývá právě brnění končetin, ale při určitých stavech se může dostavit i brnění hlavy nebo brnění obličeje.

Příčinou mravenčení bývá utlačení, poškození nebo podráždění nervů, které se starají o přenos signálů z mozku do cílových tkání. Pokud je daný nerv utlačen, nervové vzruchy nemohou probíhat a funkce končetiny je omezena. Navenek to pocítíme jako znecitlivění spojené s brněním. Důležité je rozlišovat, jak dlouho tento stav trvá a zda je spojen s dalšími klinickými příznaky. [1, 2, 3]

Možné příčiny mravenčení

Škála příčin je u brnění poměrně pestrá. Může jít o:

  • dlouhé setrvávání v jedné pozici,
  • špatné zásobení cév v důsledku aterosklerózy či omrznutí,
  • kouření,
  • nadměrné pití alkoholu,
  • užívání některých léků,
  • nedostatek vápníku, hořčíku a jiných minerálů,
  • nedostatek vitamínů E, B1, B6 a B12,
  • otrava některými chemickými látkami a toxiny,
  • podstupování chemoterapie či radioterapie.

Také může jít o jeden z projevů určitých chorob a patologických stavů:

Brnění rukou

Mravenčení postihuje velice často horní končetiny. Konkrétně si pacienti běžně stěžují na brnění prstů či brnění konečků prstů, což může poukazovat na syndrom karpálního tunelu. Mravenčení rukou se může dostavit například i při hyperventilaci, pokud má pacient záchvat paniky. Potíže odezní prakticky hned jak se pacient uklidní.

Pozor na brnění levé ruky a jedné poloviny těla

Brnění v levé ruce by se nemělo v žádném případě podceňovat, zejména pokud se objeví náhle, bez zjevné příčiny a je navíc provázeno intenzivní bolestí a pícháním. Jedná se totiž o jeden z typických příznaků infarktu myokardu.

Mimořádnou obezřetnost vyžaduje i náhlé brnění jedné poloviny těla spojené s částečným ochrnutím a dalšími příznaky. V těchto případech se může jednat o cévní mozkovou příhodu (mrtvici), kdy se hraje o čas a každá sekunda je důležitá. Čím dříve je pacientovi poskytnuta pomoc, tím lepší je prognóza. [8, 9, 10, 11]

Brnění rukou v noci

U některých pacientů se objevuje mravenčení rukou specificky jen v noci. Může jít o důsledek přeležení, kdy dotyčný na dané končetině usne. Po probuzení se dostaví nepříjemné znecitlivění a brnění, velice rychle ale odezní. Další možnou příčinou je nedostatečné prokrvení periferních částí těla.

Někdy se tímto způsobem projevuje syndrom karpálního tunelu, kdy brní hlavně palec, ukazováček a prostředníček. Potíže se postupně zhoršují, přecházejí do bolestí a ruka začíná brnět i přes den. [12, 13, 14]

Brnění nohou

Druhou nejčastěji postihovanou částí těla jsou dolní končetiny. Mezi běžné příčiny brnění nohou patří nejrůznější neurologické stavy, diabetická neuropatie či poruchy štítné žlázy. V závažných případech může jít také o příznak selhávání jater a ledvin.

Pokud se u vás objeví náhlé brnění palců u nohou či brnění chodidel, není nutné hned propadat panice. Může jít o dočasný projev nedostatku hořčíku, což se dá snadno řešit úpravou jídelníčku či podáváním doplňků stravy.

Brnění nohou v těhotenství

Těhotenství je pro některé nastávající maminky obdobím, kdy se poprvé setkávají s častějším brněním v dolních končetinách, zejména lýtkách. Rostoucí děloha může utlačovat některé nervy v pánevní oblasti, proto se tento problém vyskytuje hlavně v druhém a třetím trimestru.

Rychlý nárůst tělesné hmotnosti, změna těžiště a hormonální změny mohou taktéž přispívat k útlaku některých nervů, proto se objevuje brnění a mravenčení končetin. Po porodu tyto nepříjemné potíže většinou samy vymizí. [15, 16, 17, 18]

Mravenčení v obličeji

Parestézie postihující obličejovou část se mohou lišit svou intenzitou a rozsahem. Tyto faktory sehrávají nesmírně důležitou roli při diagnostice. Lékaře bude zajímat, zda se u pacienta objevuje například čistě jen brnění jazyka či brnění rtů, nebo jde o postižení celého obličeje či jeho poloviny. Brnění v těchto oblastech může vyvolat:

  • migréna,
  • poškození nervů traumatem, cukrovkou či jiným onemocněním,
  • záchvat paniky s hyperventilací,
  • alergická reakce (často na potraviny, léky či pyly),
  • roztroušená skleróza,
  • cévní mozková příhoda,
  • tranzitorní ischemická ataka (TIA),
  • fibromyalgie. [19, 20, 21]

Kdy vyhledat kvůli brnění lékaře?

Občasné či mírné mravenčení způsobené přeležením či úzkostí nevyžaduje lékařskou pomoc. Naproti tomu existují stavy, které by rozhodně neměly být banalizovány a u kterých je rychlá odborná pomoc nezbytností. Lékaře vyhledejte v případě, že je brnění dlouhodobé a zhoršuje se. Mezi varovné signály patří také tyto příznaky:

  • náhlá slabost,
  • problémy s řečí a orientací,
  • potíže s viděním,
  • ztuhlost jedné strany těla,
  • ochrnutí,
  • neschopnost hýbat končetinou,
  • náhlá silná bolest hlavy,

Co může způsobit brnění v levé ruce?
Občasné brnění či mravenčení ruky může mít méně závažnou příčinu, objevuje se například při karpálním tunelu. Pokud vám ale začne náhle brnět celá levá ruka a dostaví se bolest, která vystřeluje zpoza hrudní kosti do krku, mezi lopatky či do levé ruky, může se jednat o symptom infarktu myokardu.
Co to znamená, když se objeví mravenčení nohou?
Brnění dolních končetin může poukazovat na nedostatek hořčíku, který se dá snadno doplnit do jídelníčku. Také může jít o závažnější příčiny a onemocnění, jako jsou cukrovka, neurologické choroby, hypotyreóza, selhávání jater a ledvin či nádorová onemocnění.
Co pomáhá na brnění rukou?
Léčba mravenčení ruky by se měla odvíjet od příčiny. Pokud je způsobeno přetížením, přeležením, nedostatkem hořčíku či stresem, obvykle postačí ke zlepšení stavu dostatek odpočinku a úprava jídelníčku. U karpálního tunelu či uskřípnutí meziobratlové ploténky se přistupuje k podávání léků a fyzioterapii. Závažnější onemocnění, jako jsou roztroušená skleróza či rakovina, vyžadují specifickou léčbu.
