Furunkl je bolestivý hnisavý zánět vlasového folikulu, který se může objevit při proniknutí bakterií do hlubších vrstev kůže. Nejprve vypadá jako červený pupínek, postupně se začne zvětšovat a plnit hnisem. Obsah furunklu se nesmí vymačkávat. U méně závažných infekcí většinou postačí antibiotická mastička, u rozsáhlejších je nutné provést drenáž a nasadit systémová antibiotika.

Furunkl je hnisavá kožní infekce postihující vlasový folikul a jeho okolí. Česky se označuje jako nežit. Nejčastější příčinou vzniku furunklu je zanesení bakterií do hlubších vrstev kůže, konkrétně se jedná hlavně o stafylokoky, méně často o streptokoky. U zdravého člověka se tyto mikroorganismy nachází na povrchu kůže a sliznic, aniž by způsobovaly nějaké zdravotní problémy.

Pokud dojde k narušení integrity kůže (například říznutím či škrábnutím), mohou se bakterie dostat do podkoží či k vlasovým folikulům. Imunitní systém pak reaguje na přítomnost patogenů a vzniká zánět. [1, 2, 3, 4]

Furunkl a karbunkl

Kromě furunklu se ještě můžete setkat s pojmem karbunkl. Je podstatně vzácnější než furunkl, větší a má několik otvorů, kterými odtéká hnis. Jedná se vlastně o shluk několika furunklů. Karbunkly bývají lokalizovány často na zádech a v oblasti šíje. Někdy se ale může vytvořit hluboký absces, ve kterém se hnis hromadí a neodtéká.

Rozsáhlejší karbunkly bývají doprovázeny příznaky typickými pro systémové infekce, jako jsou horečka, zduření mízních uzlin, nevolnost, únava či malátnost. Karbunkl je obvykle nutné léčit systémovými antibiotiky a odbornou drenáží. Na místě zhojení většinou zůstává nevzhledná jizva. [5, 6, 7]

Projevy a průběh furunklu

V první fázi infekce vypadá furunkl jako tuhý zarudlý pupínek. Bulka je na omak teplá a bolestivá, na vrcholu se tvoří žlutobílá čepička. V průběhu několika dní se furunkl naplní hnisem. Může dosahovat různých rozměrů – u některých pacientů může jít o útvary o velikosti golfového míčku. Furunkl bývá opravdu hodně bolestivý.

Vůbec největší bolest pociťují pacienti zpravidla těsně před tím, než bulka praskne. Menší útvary se ale mohou někdy vstřebat i bez prasknutí. Furunkly i karbunkly se hojí zjizvením. Hojení běžně trvá i 3 týdny. [8, 9, 10]

Kde se objevuje furunkl?

Furunkly se objevují hlavně na místech, která jsou pokryta vlasy či chlupy, případně v kožních záhybech, kde se člověk hodně potí. Zvýšené tření spolu s pocením vytváří ideální prostředí pro pomnožení bakterií. Nejčastěji si pacienti stěžují na:

  • furunkl na genitáliích,
  • furunkl v tříslech,
  • furunkl na zadku,
  • furunkl na prsu,
  • furunkl na zádech,
  • furunkl na hrudi,
  • furunkl v obličeji,
  • furunkl v uchu,
  • furunkl ve vlasech.

U jedinců s výrazně oslabeným imunitním systémem může dojít k furunkulóze, což je termín pro mnohočetný výskyt furunklů různě po těle. Typickým příkladem jsou třeba pacienti s cukrovkou. [11, 12, 13]

Rizikové faktory furunklu

Vznikem furunklů jsou ohroženi především pacienti s kožními chorobami a lidé s oslabenou imunitou. Náchylnější k těmto infekcím jsou také senioři a dospívající jedinci. Dalším předpokladem ke vzniku furunklů je nedostatečná hygiena. Mezi rizikové faktory furunklu tedy patří:

  • kožní onemocnění (lupénka, ekzém, cystické akné apod.),
  • diabetes,
  • onemocnění AIDS,
  • onkologická onemocnění,
  • podvýživa,
  • užívání léků ovlivňujících imunitní systém (nejčastěji kortikoidy),
  • nadváha a obezita,
  • nadměrné ochlupení,
  • nedostatečná osobní hygiena. [14, 15, 16]

Furunkl: léčba

Některé drobnější furunkly se mohou samovolně vyléčit. Pokud ale bulka přetrvává déle než týden, je tvrdá, bolestivá či se zvětšuje, bez lékařského ošetření se neobejdete. Jestliže lékař rozhodne, že stačí lokální léčba, postačí vám antibiotická mast na furunkl. Důležité je dbát i na hygienu rány, když furunkl praskne. K úlevě od intenzivní bolesti pomohou analgetika.

Léčba furunklu o větší velikosti už bývá náročnější. Lékaři obvykle přistupují k drenáži, což je naříznutí furunklu za lokálního znecitlivění a vypuštění infekčního obsahu. Ložisko je nutné pečlivě vyčistit, odstranit veškerou hnisavou tkáň a vydezinfikovat. Po tomto zákroku pocítí pacient úlevu.

Ošetřující lékař mu po zákroku předepíše antibiotika, která je nutné poctivě užívat přesně podle pokynů. Pacient musí chodit ještě několik dní po drenáži na převazy. A samozřejmostí je opět nezbytná pečlivá hygiena rány. [17, 18, 19, 20]

Komplikace furunklu

V žádném případě se nedoporučuje hnis z furunklu vymačkávat či ho nějak podomácku propichovat. Bakterie se mohou dostat do okolních tkání, vytvořit absces, napadnout okolní orgány či způsobit sepsi. Velice nebezpečné jsou v tomto ohledu furunkly na hlavě a ve vlasech. Bakterie v této oblasti mohou způsobit hnisavý zánět mozkových blan nebo trombózu kavernózního splavu.

Dostane-li se infekce do oběhu, může putovat krevním řečištěm po celém těle a napadat nejrůznější tkáně a orgány. Mezi vážné komplikace furunklu patří:

  • endokarditida (zánět vnitřní blány srdeční),
  • osteomyelitida (zánět kostí),
  • artritida (zánět kloubů),
  • empyém (tělesná dutina naplněná hnisem),
  • flegmóna (neohraničený zánět měkkých tkání).

Karbunkly i furunkly jsou vysoce nakažlivé, proto se může infekce přenést i na další členy domácnosti. [21, 22, 23]

Prevence furunklu

Zanesení infekce do otevřené rány na kůži se dá předcházet pečlivou hygienou a dodržováním určitých opatření. Pokud proděláváte stafylokokovou infekci (nebo někdo z vašich blízkých), doporučuje se:

  • časté mytí rukou,
  • pečlivá péče o jakékoliv rány a škrábance,
  • pravidelné praní ložního prádla na vysokou teplotu,
  • nošení volného a vzdušného oblečení,
  • posilování imunitního systému (pestrý jídelníček, pohyb, eliminace stresu),
  • dodržování léčby přidruženého chronického onemocnění (jste-li diabetik, atopik apod.). [24, 25, 26]
