Neplodnost u mužů většinou nesouvisí s problémy s erekcí, ale týká se především problému spermií. Faktorů, které neplodnost způsobují, existuje celá řada, z toho důvodu je důležité nejprve pátrat po příčině a až následně přistoupit k léčbě a alternativním metodám reprodukce. Kromě zvýšené teploty varlat, alkoholu a kouření mohou být příčinou i různá onemocnění, v jiných případech hrají roli genetické a imunologické faktory. 

Pravděpodobnost neplodnosti u mužů

Světová zdravotnická organizace (WHO) uvádí, že neplodnost u mužů bývá častější příčinou problému počít dítě než u žen, přestože se dříve problémy hledali spíše u žen. Jenže zatímco ty bývají neplodné v 25 % případů, u mužů je to 33 %. Ve 20 % situací pak dochází k neplodnosti ze strany obou partnerů, což znamená, že mužská neplodnost se podílí na nemožnosti počít až u poloviny případů. U 15 % párů se důvod neplodnosti nepodaří odhalit. Faktorů, které mohou ovlivnit neplodnost u mužů, existuje celá řada.

Příčiny neplodnosti u mužů

Příčiny mužské neplodnosti lze rozdělit na 3 základní typy, nicméně mužská plodnost bývá rovněž ovlivňována imunologickými a genetickými faktory.

  • Testikulární příčiny
  • Posttestikulární příčiny

Pretestikulární příčiny

Pretestikulární příčiny neplodnosti znamenají, že u mužů nedošlo k dostatečnému vývoji mužských pohlavních žláz, tedy varlat. Důvody bývají různé, mohou být způsobeny genetickými odchylkami, stejně tak ale může být viníkem příliš těsný oděv, jízda na kole či na koni, dlouhé sezení v autě a zvýšená teplota varlat zapříčiněná horkými koupelemi, častými návštěvami sauny, prací na notebooku na klíně či vyhřívanými sedačkami v autě.

Testikulární příčiny

V tomto případě se jedná o stav, při kterém je neplodnost způsobena nekvalitním ejakulátem, konkrétně nízkým počtem pohyblivých a životaschopných spermií, dále mohou být spermie vadné z hlediska tvaru či složení, vyloučená není ani úplná absence spermií v ejakulátu.

Pokud jsou spermie vadné, hovoříme o tzv. teratospermii, při nedostatečném počtu životaschopných spermií se setkáváme s tzv. oligospermií a v případě úplné absence spermií v ejakulátu dochází k tzv. azoospermii.  Nekvalitní ejakulát může být zapříčiněn řadou faktorů, vliv na kvalitu spermií má zejména častější pití alkoholu a kouření a také různá onemocnění.

Nemoci ovlivňující neplodnost u mužů

Mezi nemoci, které ovlivňují neplodnost u mužů, patří například spalničky nebo příušnice prodělané v dospělosti. Neštovice v dospělosti neplodnost u mužů nezpůsobují, jak se někteří lidé mylně domnívají. Mezi nejčastější testikulární příčiny neplodnosti řadíme následující nemoci:

  • Chlamydie
  • Diabetes
  • Malárie
  • Nádory
  • Porucha funkce štítné žlázy
  • Příušnice a spalničky v dospělosti

Posttestikulární příčiny

V případě, že se jedná o posttestikulární příčiny, problém spočívá v průchodu spermií z těla ven, zatímco funkce mužských pohlavních žláz je v pořádku. Nejčastějšími důvody jsou neprůchodnost chámovodů, absence chámovodů, zánět prostaty, chybné vyústění močové trubice nebo impotence.

Genetický a imunologický faktor

Genetický faktor se vztahuje k mužům, kteří jsou přenašeči genetických vad, z toho důvodu se obvykle přistupuje k mimotělnímu oplodnění, aby mohla být genetická vada vyloučena. Tento faktor zároveň může mít vliv na neschopnost otěhotnět přirozenou cestou.

Imunologické problémy mohou zapříčinit napadení spermií imunitním systémem. Imunitní systém produkuje protilátky, které zhoršují pohyblivost spermií, jejich schopnost oplodnit vajíčko, ale může mít také na svědomí samovolný potrat. Oba dva faktory mohou být příčinou i neplodnosti u žen.

Jak poznat neplodnost u muže?

Pokud ani po roce pravidelného nechráněného pohlavního styku nedochází k početí potomka, je velmi pravděpodobné, že se jeden z partnerů potýká s neplodností nebo se sníženou možností počít. Jaké má dále neplodnost u mužů příznaky? Projevit se může například neschopností ejakulovat, případně při pohlavním styku uvolněním pouze malého množství ejakulátu.

Jak zjistit neplodnost u mužů vyšetřením?

Vzhledem k vyšší pravděpodobnosti mužské neplodnosti oproti ženám se nejdříve vyšetřují muži. K základnímu vyšetření spermatu se využívá spermiogram, dále se přistupuje k vyšetření hormonálního prostředí, poté je možné dále přistoupit k vyšetření specializovaným lékařem.

Následná vyšetření se odvíjejí od výsledků předcházejících testů. Pokud budou výsledky z vyšetření spermatu v pořádku a neprokážou se známky neplodnosti, na řadu přichází vyšetření neplodnosti u ženy. Bez důkladného vyšetření a následné léčby se nedoporučuje přistupovat k alternativním metodám reprodukce.

Léčba mužské neplodnosti

Pokud je příčina mužské neplodnosti známá, při léčbě se nejprve přistupuje k odstranění dané příčiny. Pokud je důvodem nějaký zánět, léčba se řeší pomocí antibiotik. Další možností je vyrovnání hladiny hormonů a v některých případech je možný mikrochirurgický zákrok.

Je-li důvodem k nemožnosti počít zhoršená kvalita spermií, přistupuje se k oplodnění vajíčka ve zkumavce nebo mikroinjekcí spermie do ženského vajíčka. Pokud muži trpí úplnou absencí spermatu, je možné mikrochirurgickou operací získat sperma přímo z varlete či nadvarlete.

Ke zlepšení kvality spermií je možno přispět omezením kouření, alkoholu a nadměrné fyzické zátěže, předcházením zvýšení teploty pohlavních žláz a zařazením vitamínových doplňků do stravy, zejména vitamínů E a C.
