Náhlé vzniklé intenzivní bolesti v jedné části obličeje, které se vracejí i několikrát za den, bývají způsobeny zánětem trojklanného nervu, stavem zvaným také neuralgie. Příčinou může být komprese cévy, ale i závažnější situace, jako je mozkový nádor nebo roztroušená skleróza, a proto byste návštěvu lékaře raději neměli příliš oddalovat. Jaká je léčba?

Trojklanný nerv, odborně zvaný nervus trigeminus, je párový nerv, který má tři základní větve. Tyto větve mají zakončení v kůži obličeje, odkud se sbíhají a následně vstupují drobným otvorem v lebce až do mozkového kmene. Hlavní funkcí trojklanného nervu je pak přenos informací o citlivosti z poloviny obličeje.

Obtíže spojené s tímto nervem mohou představovat jednu z nejhorších bolestí, které člověk může zažít. Zánět trojklanného nervu, odborně nazývaný neuralgie trojklanného nervu, je stav definovaný intenzivní bolestí obličeje, která může narušovat základní činnosti, jako je mluvení, úsměv, čištění zubů a podobně. Bolest se obvykle projevuje v krátkých a intenzivních epizodách, které se vrací, ale výjimečně může být neustálá a méně prudká. [1, 2, 3, 4]

Zánět trojklanného nervu: příznaky

Neuralgie trojklanného nervu je častější u žen než u mužů a také více postihuje lidi starší čtyřiceti let. Obvykle se projevuje záchvaty, které mohou trvat od několika sekund až do dvou minut a opakovat se několikrát denně v různých intervalech.

Zánět trojklanného nervu postihuje zpravidla jednu stranu obličeje a projevuje se v oblasti oka, tváře, ucha a dolní čelisti, čímž může připomínat i bolest od zubu. Typicky tato bolest bývá prudká, elektrizující a někdy až nesnesitelná, ale výjimečně se může jevit také jako tupá. Mezi typické příznaky patří:

  • bodavá bolest připomínající elektrický šok,
  • bolest postihující pouze jednu stranu obličeje,
  • záchvaty bolesti vyvolané dotykem obličeje, mluvením, žvýkáním a podobně,
  • intenzivní bolest přetrvávající od několika sekund do několika minut,
  • bolest mohou doprovázet křeče v obličeji,
  • záchvaty trvají dny, týdny i měsíce,
  • bolest může být centralizovaná v jednom místě, ale i rozšířená v celé části obličeje,
  • bolest se obvykle neprojevuje v noci,
  • bolest mohou doprovázet poruchy citlivosti kůže nebo sliznice.

Atypická neuralgie trojklanného nervu

Atypická neuralgie trojklanného nervu je méně bolestivá a přetrvává dlouho, ale obvykle bývá více rozšířená. Můžete pociťovat píchání nebo pálení v jedné části obličeje doprovázené tupou bolestí. U tohoto typu zánětu trojklanného nervu může být obtížnější rozpoznat příznaky. [5, 6, 7, 8, 9]

Zánět trojklanného nervu: příčina

U zánětu trojklanného nervu je zpravidla narušena jeho funkce. Obvykle nastává problém mezi normální krevní cévou, tedy žílou nebo tepnou, a trojklanným nervem na spodině vašeho mozku. Tento kontakt může vyvíjet tlak na nerv a způsobuje jeho poruchu. Komprese cévou je jednou z nejčastějších příčin bolesti, ale důvody mohou být i jiné a závažnější. Někdy tyto bolesti ale mohou mít nejasný původ. Možné příčiny dále zahrnují následující:

Diagnóza neuralgie je založena na anamnéze pacienta a jeho konkrétních obtížích. Pokud se potýkáte s výše popsanými bolestmi, váš praktický lékař by vás měl nejdříve vyšetřit a následně poslat na neurologické vyšetření. Mohou vám být doporučeny některé zobrazovací a laboratorní testy, jako je CT a magnetická rezonance s vysokým rozlišením trojklanného nervu.

Tato vyšetření mohou pomoci odhalit, zda jsou vaše problémy způsobeny kompresí cévy, nebo je mají na svědomí jiné stavy, jako je nádor, aneurysma nebo roztroušená skleróza. Může se stát, že testy přesnou příčinu bolesti nezjistí, ale mohou alespoň vyloučit závažnější problémy. [14, 15, 16]

Zánět trojklanného nervu: léčba

Léčba zánětu trojklanného nervu závisí především diagnostice příčiny. Často se využívají léky, které se předepisují pro kontrolu záchvatů, jako jsou antiepileptika a antidepresiva. Správné dávkování vždy určí váš ošetřující lékař. Tato léčiva mohou být případně kombinována s některými léky proti bolesti. Kvůli možným vedlejším účinkům mohou být při užívání těchto léčiv také vyžadovány pravidelné krevní testy.

Dále je možné využít fokusovanou rázovou vlnu pro stimulaci nervu a udržení běžného stavu měkkých tkání. Pokud léčba není dostatečně účinná nebo jsou vedlejší účinky léků příliš rizikové pro vaše zdraví, další možnost léčby představuje neurochirurgický zákrok. Během operace jsou odstraňovány struktury, které způsobují stlačení nervů.

Kromě konvenční léčby neuralgie můžete jako doplňkovou léčbu vyzkoušet i některé přírodní metody, jako je akupunktura, chiropraxe nebo bylinky. Pokud již podstupujete léčbu zánětu trojklanného nervu a uvažujete o bylinkách nebo homeopatikách, poraďte se ale vždy nejprve s vaším ošetřujícím lékařem, protože mohou ovlivnit účinky léků. Mezi bylinné produkty, které mohou mít podpůrný účinek, patří zejména mátový olej a levandule. [17, 18, 19, 20]
