Systémový lupus erythematodes, či zkráceně SLE, je závažné autoimunitní onemocnění, kterým trpí převážně ženy. Může postihnout pouze kůži, ale v mnoha případech jsou zasaženy také klouby, srdce, plíce a ledviny. Projevy SLE se dají jednoduše zaměnit i s jinými nemocemi, což u pacientů komplikuje správné určení diagnózy.


Kožní lupus je velmi nepříjemné a omezující onemocnění, které si pacient s sebou nese po celý život. Slovo lupus znamená latinsky vlk a má původ ve středověku, kdy projevy onemocnění lékařům připomínaly stopy vlčího kousnutí. V České republice touto nemocí trpí téměř deset tisíc lidí. Ženy jsou postiženy mnohem častěji než muži, zhruba v poměru 9:1.

Přirozeným úkolem imunitního systému je chránit lidský organismus. V případě lidí, u kterých se projevil systémový lupus, to tak ovšem nefunguje. Jejich imunitní systém naopak útočí na vlastní buňky, bojuje proti vlastním tkáním a orgánům, způsobuje v nich zánět a ničí je. Lupus může způsobit vážné komplikace, ale také úplné selhání životně důležitých orgánů.

Přesná příčina systémového lupusu prozatím není známá. Na vzniku tohoto onemocnění se však s největší pravděpodobností podílí kombinace genetických, hormonálních a vnějších faktorů, mezi které patří sluneční záření, některé infekce a léky, stres a podobně. Pro okolí tato nemoc ovšem není nakažlivá.

Příznaky lupusu

Kožní lupus je známý tím, že se může projevit celou řadou příznaků, které se často u každého pacienta liší. První projevy se obvykle objevují mezi 15 a 44 lety. Někteří lidé pociťují pouze mírné nepříjemnosti, u jiných však postižení může vést až k ohrožení života.

Nejznámějším příznakem je takzvaný motýlovitý erytém. Jedná se o zarudnutí na kůži obličeje v typickém tvaru motýlích křídel. Vyrážku ale můžete pozorovat i na ramenou, hrudníku a horních končetinách. Tyto kožní projevy se často zhoršují také vlivem slunečního záření.

Další projevy:

  • Vypadávání vlasů
  • Velká únava
  • Úbytek na váze
  • Bolesti a otoky kloubů
  • Bělání a fialovění prstů v chladu
  • Dušnost a bolesti na hrudi
  • Zvýšená náchylnost k infekcím
  • Problémy se zrakem
  • Zhoršení paměti
  • Změny chování či snížení inteligence

V průběhu onemocnění se mohou projevy lupusu měnit. Často se střídá období zhoršení nemoci (relaps) s obdobím, kdy je lupus méně aktivní a příznaky jsou mírnější nebo úplně vymizí (remise).

Lupus a možné komplikace

Kromě již zmíněných příznaků může lupus způsobit také zánět ledvin neboli lupusovou nefritidu. Naruší se tak schopnost ledvin odvádět z těla zplodiny metabolismu, v moči se při zánětu vyskytne krev a bílkovina. Pacient pak často trpí otoky a hrozí až selhání ledvin. Někdy se ovšem stane, že postižení ledvin nedoprovází žádné obtíže, a poté je třeba pro jistotu vyšetřit moč a odebrat vzorek tkáně z ledvin.

Mezi další možné komplikace spojené s výskytem lupusu patří například onemocnění srdce a cév, onemocnění plic, chudokrevnost nebo také deprese a úzkosti.

Diagnostika lupusu

Správně stanovit diagnózu lupusu není vůbec jednoduché. Lupus totiž postihuje více orgánů a příznaky často naznačují, že se jedná o zcela jiné onemocnění.

Neexistuje jeden konkrétní test, díky kterému by bylo možné potvrdit podezření, že se jedná právě o lupus. Lékaři často využívají kombinaci různých metod, mezi které patří podrobné fyzikální vyšetření, laboratorní testy (imunologické testy, biochemické vyšetření, krevní obraz…), oční a neurologické vyšetření a také kožní či renální biopsie.

Jak lupus léčit

V současné době nelze lupus nijak zcela vyléčit. Existuje však řada možností, které pomáhají zmírnit průběh onemocnění. Celkovým cílem léčby je kromě navození stavu remise (zmírnění nebo odstranění příznaků nemoci) také prevence a zabránění trvalému poškození orgánů.

Léčba lupusu se u jednotlivých pacientů liší podle rozsahu jejich onemocnění. Většinou se podávají imunosupresiva, což jsou léky, které potlačují imunitní systém. Jejich vedlejším efektem je ale nádorové bujení, protože potlačený imunitní systém proti němu nemůže tělo bránit.

Další možností léčby jsou kortikoidy (například Prednison), které sice působí protizánětlivě, ale mají také celou škálu vedlejších účinků, nesteroidní antirevmatika (například Ibuprofen nebo Diclofenac) a antimalarika.

Co může udělat sám pacient:

  • Chránit pokožku před sluncem
  • Nekouřit
  • Pravidelně cvičit
  • Vyhýbat se stresu a infekčnímu prostředí
  • Pravidelně docházet na lékařské kontroly
  • Informovat lékaře o nových příznacích a vedlejších účincích léků

Lupus v těhotenství

Těhotenství a mateřství může být pro pacientky s lupusem velmi problematické. Často kvůli komplikacím spojeným s tímto onemocněním vůbec otěhotnět nemohou nebo u nich opakovaně dochází k potratům. Pokud se nemoc přenese z matky na plod, u novorozenců se lupus projeví nejčastěji na kůži a na srdci.

Kvůli doprovodným rizikům, jako jsou různé komplikace či nežádoucí účinky léků na lupus, je nutné těhotenství pečlivě naplánovat a následně ho sledovat také na specializovaném pracovišti. Ženám se obvykle doporučuje pokusit se o miminko v době, kdy od posledního relapsu nemoci uběhlo více než půl roku.

Dobrý den,
příčin může být hned několik.
Mezi nejčastější příčinu patří špatné stravovací návyky - nedostatek vitamínů (A, B6, B12, vit. C, biotin,…), nedostatek stopových prvků (Zn, Fe, Mg,…). V tomto případě bych jako první změnila stravovací návyky - zdravá a pestrá strava. Dále existují spousty volně prodejných doplňků stravy - pro příklad uvádím Revihair od Skinexpert, Biosil, …
Nepíšete, kolik vám je let. Váš problém může souviset také s hormonální dysbalancí během menopauzy.
V lékárnách můžete opět zakoupit spousty přípravků proti vypadávání vlasů souvisejícím s menopauzou, ať už se jedná o šampony, či o různá séra - většinou na bázi fytohormonů. Vlasy mohou padat také po porodu.
Dalšími příčinami jsou např. nadměrný stres nebo také častá úprava vlasů ve smyslu žehlení/foukání.
Příčiny mohou být samozřejmě také kožní - např. psoriasa (lupenka) v oblasti pokožky hlavy, …. Vypadávání může souviset samozřejmě také s nějakou interní nemocí (např. hyperfunkce štít. žlázy, ….)

V první řadě bych zkusila odbourat stres (pokud nějaký máte), zaměřila bych se na zdravou a pestrou stravu, příp. bych zakoupila volně prodejné doplňky stravy či šampony/sera. Pokud během několika měsíců neuvidíte zlepšení (které 100% neuvidíte hned, ale nejdříve tak za 2-3 měsíce), pak bych zvážila návštěvu lékaře, který vás podrobně vyšetří, případně pošle k dalším odborníkům.
